1. Determine your membership status – If you are a non-member, please use the non-member pricing options listed. Incorrectly submitted registrations will delay your processing or will result in non-registration.

2. Determine the number of days you will be attending the seminar - If you will be attending both Friday, September 12 and Saturday, September 13, please fill out the fields at the top of the form. For one-day registrants, please use the middle section.

3. If you are attending only one day and want to attend the Saturday banquet - Those registrants who wish to attend the Saturday evening banquet but are attending only one-day will be charged an additional fee and must fill out the registration form appropriately.

4. Payment options - Payment may be made by check or credit card utilizing this registration form. For wire transfers, please contact the IASC office (301.588.1171 x102). Credit card information transferred over the internet is not secure. Registrants may print/fax registrations to 301.588.1174.

5. Cancellations - Registration cancellations prior to August 28, 2008 will be granted a full refund less a $75 processing fee. No refunds after August 28.
